The hook on the top left in the photo is designed for challenging tire installs. The hole in the handle is designed to protect your knuckles in case you pop loose and punch the spokes. This is a specialist tool as far as I'm concerned. For almost every tire and tube, I prefer Park's levers: http://www.pricepoint.com/Brand/Park-Tool/Park-Tool-TL-1-2C-Tire-Lever-Set.axd But, when you have one of those super tough fits, this can be a useful tool in the quiver. Just be careful not to pinch the tube in the process.
